To
Order:
1) Left-click on the
appropriate link for the
division you need; it
should open the form.
Alternatively, you may
right click on the
appropriate button and
select "Save Target
As," and then save
the form to your desktop.
2) Fill out the form by
changing the
"0's" in the
boxes to the number of
pieces of each item you
need.
3) After you have finished
filling out your order
form, Save it to your
desktop.
4) You can submit your
order one of two ways: by
email or fax. Please
include your name, phone
number, and attach the
order form you saved to
the desktop in the e-mail,
or you can print the sheet
and fax it to: (940)365-0073.
If you would prefer to
email the form, please
send the email to: devinc@bluemoonsportswear.com
and be sure to attach the
order form to your email.
5)
CAUTION! We
periodically update our
order forms to add new
items and sizes, so be
sure to check that you are
using the most current
order form when you place
an order! (Don't assume
that a form you saved on
your desktop when you
placed your last order is
still the current form!)
